Resumo

Introdução: A obesidade é uma doença crónica, de origem multifatorial, associada a diversas complicações médicas, provocando um declínio significativo na saúde dos indivíduos. Em indivíduos com IMC≥40kg/m² ou IMC≥35kg/m² com, pelo menos, uma comorbilidade, a cirurgia bariátrica é o tratamento de eleição e pode ser realizado em cirurgias do tipo restritivas e/ou malabsortivas.

Objetivos: Realizar uma revisão da literatura para estudar a evolução ponderal após cirurgia bariátrica a curto e a longo prazo.

Metodologia: foram selecionados 10 artigos das bases de dados Web Of Science e Pubmed/Medline, publicados entre 2015 e 2020, que abordassem a evolução ponderal após cirurgia bariátrica. Para a realização da revisão sistemática foram utilizadas as recomendações PRISMA, e para avaliação da qualidade dos artigos o instrumento de avaliação crítica de Crombie.

Resultados: Em 90% dos artigos analisados, verificou-se uma perda de peso nos primeiros dois anos de seguimento, constatando--se ainda, em 40% dos artigos uma maior diminuição de IMC, no primeiro ano após cirurgia bariátrica comparativamente com o segundo, sendo que a diminuição média de IMC variou entre 9,62kg/m² e 19,8kg/m² no primeiro ano e entre 0,13kg/m² e 4,3kg/m² no segundo ano após cirurgia bariátrica. 30% dos artigos com um tempo de seguimento superior a 2 anos, incluídos nesta revisão, referem novo aumento ponderal dos indivíduos ao fim de 3 anos e 10% após 5 anos de cirurgia bariátrica. Dos artigos que fazem menção a mais do que uma técnica cirúrgica (40%), o Bypass Gástrico em Y de Roux é a que se relaciona com maior perda ponderal, seguida de Gastrectomia Vertical/Gastrectomia Vertical por Laparoscopia e com menores resultados a Banda Gástrica Ajustável/Banda Gástrica Ajustável por Laparoscopia.

Conclusões: A cirurgia bariátrica continua a ser um método eficaz para o tratamento da obesidade mórbida ou obesidade grau 2 com pelo menos uma comorbilidade a curto prazo, sendo necessário o acompanhamento nutricional rigoroso para ajudar na obtenção de melhores resultados a longo prazo, bem como na sua manutenção.

 

 

 


Abstract






Introduction: Obesity is a chronic disease of multifactorial origin, associated with several medical complications, causing a significant decline in the health of individuals. In individuals with BMI≥40kg/m² or BMI≥35kg/m² with at least one comorbidity, bariatric surgery is the treatment of choice and can be performed in restrictive and/or malabsorptive surgeries.


Objectives: To carry out a literature review to study short-term and long-term weight gain after bariatric surgery.


Methodology: 10 articles were selected from the Web Of Science and Pubmed/Medline databases, published between 2015 and 2020, that addressed changes after bariatric surgery. To carry out the systematic review, the PRISMA recommendations were used, and to assess the quality of the articles, the critical evaluation instrument by Crombie.


Results: In 90% of the articles analyzed, there was a weight loss in the first two years of follow-up, with 40% of the articles also showing a greater decrease in BMI in the first year after bariatric surgery compared to the second, with the mean BMI decrease varied between 9.62kg / m² and 19.8kg / m² in the first year and between 0.13kg / m² and 4,3kg / m² in the second year after bariatric surgery. 30% of the articles with a follow-up period of more than 2 years, included in this review, refer to a new weight increase in individuals after 3 years and 10% after 5 years of bariatric surgery. Of the articles that mention more than one surgical technique (40%), Roux-en-Y Gastric Bypass is related to greater weight loss, followed by Sleeve Gastrectomy / Laparoscopic Sleeve Gastrectomy and with lower results the Adjustable Gastric Band / Laparoscopic Adjustable Gastric Band.


Conclusions: Bariatric surgery remains an effective method for treating morbid obesity or grade 2 obesity with at least one short-term comorbidity, requiring strict nutritional monitoring to help achieve better long-term results, as well as in its maintenance.
